Have you visited this practice in the past 12 months AND do you identify with one of the following:
have a suspected COVID-19 infection
required to self isolate or self quarantine
under 16 years old
at least 70 years old
at least 50 years old of Aboriginal or Torres Strait Islander descent
a parent of a child under 12 months
already under treatment for chronic health conditions or is immune compromised
holds a valid concession or health care card
OR are you:
under the age of 12 months old
currently experiencing homelessness
live in a COVID-19 Impact Area (such as Victoria)
receive the care from a medical practitioner located at an Aboriginal Medical Service or an Aboriginal Community Controlled Health Service?
